Purpose of the Program

The Supporting Health for All through Reinvestment (SHARE) Initiative aims to address Social Determinants of Health and Equity (SDOH-E) by ensuring Coordinated Care Organizations’ (CCOs) profits are reinvested in their communities and improve community health by requiring reinvestments go towards SDOH-E (housing, food, transportation, educational attainment or civic engagement).

This grant award highlights SOCRH’s intention to work with SPARC Network Partners on collaborative funding streams in an effort to bring sustainable assets to our region for shared outcomes.

SOCRH anticipates applying for further SHARE funding for 2027. Check back here, join our SPARC Network Roundtable, or contact the SPARC Network Manager directly if your organization would like to collaborate with SOCRH on future SHARE Initiative funding.
